Step 1: Create an Account
I started by going to fillout.com and selecting “Get Started.” By signing up, you’ll automatically be placed on the free plan to try Fillout before committing to a paid plan!

Step 3: Select a Template
Next, I had to pick a template. There were dozens to choose from across a variety of industries.
After selecting my template, Fillout gave me a little preview of my form to see what it included. I hit “Use this template.”
Step 4: Name Your Form
The first thing I did within the editor was name my form. I did this by hitting the empty field on the top left.
Naming your form is more important than you might think. If you give your forms generic names like “Customer Survey,” you’ll struggle to find the specific one you need.

The first slide of the form was easy to edit. The left panel displayed my “Settings,” where I could edit the button text and change the image and layout.
